Clinical services commissioned

The CCG works with local hospitals, specialist health providers, GPs and other family health practitioners, local authorities, voluntary organisations and the community as a whole to make sure our services are doing the right things and are being provided from the right place. These include:

  • Acute, Community and Mental Health Hospital Foundation and NHS Trusts
  • Independent Sector Providers
  • General Practice
  • Other Independent Contractors (Pharmacists, Dentists, Opticians)
  • The County Council
  • Local Authorities
  • Third Sector and Voluntary Organisations.

Accessing personal records

To access your own medical or personal records, or to do so on behalf of a relative, (you will need their authority or permission to do so) please contact your GP.

If you require information about hospital or community care please contact the organisation who provided the care. The CCG itself does not have access to such personal information.
